INTENSIVE CASE MANAGEMENT FOR SENIORS LIVING WITH DEMENTIA

Our case managers help seniors at risk living with dementia. Our program is available for Hamilton residents who are 60 years old and over. The services include; assessment, care planning, coordination of services, education, and advocacy.

This program provides intensive case coordination to individuals with responsive behaviours relating to dementia. This service aids not only the individual being referred, but the support system of this individual, as the case coordinator will collaborate with the family and other support systems to best assist the individual. This program is designed to connect individuals to community resources, prevent and decrease hospitalization, and advocate on behalf of the individuals who are referred.  This service is free and confidential.

The ultimate goal is to improve the quality of life for client and support system by reducing responsive behaviours. 

Characteristics of clients living at high risk may include the following:

  • Lives alone
  • Resists care
  • Experiences isolation
  • Frequent visits to the emergency room
  • Feels overwhelmed


Services for those clients identified “at high risk” include the following:

  • Assessment
  • Care planning
  • Coordination of services
  • Education
  • Advocacy


Client criteria:

  • Individual must be 60 years of age and over
  • Hamilton resident
  • Living at risk and living with dementia
