INTENSIVE CARE MANAGEMENT FOR SENIORS AT RISK

Specializing in elder abuse, neglect and self-neglect. Our case managers help to support seniors, by enabling them to live independently and safely in their homes for as long as possible.

Intensive Case Management for Seniors at Risk (ICM) provides intensive case coordination with an in-depth, personalized, flexible, and creative approach. This program specializes in elder abuse, including physical, psychological/verbal, financial, neglect and/or self-neglect. Anyone may call to make a referral. The service is free and confidential.

Overview of Services

  • Case coordinators offer home visits, where a comprehensive, holistic assessment of the client’s situation is completed; then relevant, realistic, and attainable options are discussed
  • Consultation offered via telephone for families/caregivers who do not wish to receive a home visit
  • The program is client driven and client-centered
  • Cultural differences and personal preferences are always taken into consideration to preserve autonomy


Primary goals:

  • Ultimate goal is to improve the client’s quality of life by utilizing the least intrusive and most accommodating approach
  • assistance and support provided to seniors so they may live independently in the community
  • to provide education, awareness, and intervention regarding elder abuse


Client criteria:

  • 60 years of age and over
  • residing in the City of Hamilton
