Description

• Provides case management, advocacy, counselling and support to developmentally disabled adults living in the community who have little or no supports
• Promotes clients' independence through short-term counselling and emotional support, problem solving and securing life skills training.
• Helps the individual and/or family to identify, assess the suitability of, and access services which might be required including accommodation, transportation, medical/dental/financial services, vocational/leisure programs, and counselling and mental health services.
• Advocates on behalf of the client with community agencies and services to ensure that the clients' rights are acknowledged and that the clients benefit from an adequate provision of service.

Passport

Passport is an initiative of the Ministry of Community and Social Services for people with a confirmed developmental disability that have left school and want to find more ways to participate in their community. Through Passport, participants can receive funding for activities that encourage their personal development and help them achieve their potential.

Catholic Family Services have staff trained in case management that can provide assistance to participants and their families to reach their individual goals and objectives. CFS can also facilitate all Human Resource functions including payroll for staff hired by the participants.

Tue, Wed, Thu: 8:30am - 7:30pm

Eligibility-Passport is intended for residents of Ontario that have a confirmed developmental disability and:

• Have left school and would benefit from supports that will help them participate in recreational, vocational or educational opportunities in the community; or 

• Have been waiting for service and are in need of community participation supports; and

• Are not receiving Ontario Disability Support Program (ODSP) employment supports.

Participants in Passport can live independently or in supportive living. Priority will be given to applicants who are living at home with their families.

Application-Contact Hamilton is responsible for administering Passport for residents of Hamilton. Individuals and families that are interested in Passport can call Contact Hamilton at 905-570-8888. Contact Hamilton staff will send out an application form and related Passport information. A follow up appointment can be set up to address any questions relating to Passport and / or to explore other areas of need. Individuals and families can also choose to download the Passport application form and related information directly from Contact Hamilton's website.
