PassportPassport is an initiative of the Ministry of Community and Social Services for people with a confirmed developmental disability that have left school and want to find more ways to participate in their community. Through Passport, participants can receive funding for activities that encourage their personal development and help them achieve their potential.
Catholic Family Services have staff trained in case management that can provide assistance to participants and their families to reach their individual goals and objectives. CFS can also facilitate all Human Resource functions including payroll for staff hired by the participants. Hours-Mon, Fri: 8:30am - 4:30pm Areas Served-Hamilton (City of)
Eligibility-Passport is intended for residents of Ontario that have a confirmed developmental disability and:
Participants in Passport can live independently or in supportive living. Priority will be given to applicants who are living at home with their families.
Application-Contact Hamilton is responsible for administering Passport for residents of Hamilton. Individuals and families that are interested in Passport can call Contact Hamilton at 905-570-8888. Contact Hamilton staff will send out an application form and related Passport information. A follow up appointment can be set up to address any questions relating to Passport and / or to explore other areas of need.
